- Conducts health education focused on prevention, chronic disease management, and lifestyle modification.
- Conducts regular patient follow-ups via phone using CCIQ platform.
- Collaborates with providers and clinical teams for care coordination.
- Assists patients with appointment scheduling and navigating healthcare services.
- Maintains timely and accurate documentation in EHRs and other required platforms.
- Promotes medication adherence, risk reduction, and patient engagement.
- Ensures compliance with all applicable local, state, and federal care delivery regulations.
- Demonstrates proficiency in utilizing the CCIQ platform to manage and coordinate patient care, ensuring accurate documentation, follow-up, and communication across the care team.

Minimum Qualifications * Education: Graduation from an Accredited School of Nursing required, BSN preferred.
- Work Experience: Minimum 1 year of experience required. Background in case management or chronic disease care preferred.
- Special Training, Certification or Licensure: Current Colorado Registered Nurse (RN) License and current CPR/BLS certification required upon hire for all areas.
