Job Summary

A company is looking for a Clinical Pharmacist Patient Care to conduct telephonic medication consultations and improve patient health outcomes.

Key Responsibilities:

• Conduct telephonic medication consultations that meet/exceed production and quality metrics
• Complete annual medication reviews and pharmacotherapy assessments for patients
• Deliver evidence-based therapeutic recommendations and document clinical consultations

Required Qualifications:

• Active, unrestricted pharmacist license in the state of primary residence
• 3+ years of pharmacy practice experience, including 1+ years in Clinical Pharmacy Practice
• 1+ years of experience with Medicare STARs & HEDIS Measures
• Proficient in Microsoft Office Suite
• Able to manage multiple, concurrent projects in a rapidly changing environment
