Clinical Care Coordinator

Job Summary

A company is looking for a Clinical Care Management Coordinator to work from home.

Key Responsibilities

• Initiate outbound calls for care management campaigns and gather member-specific health data
• Complete Health Risk Assessments and assist members in accessing appropriate medical providers and community-based services
• Collaborate with RN Case Managers and ensure accurate documentation of all activities

Required Qualifications

• Clinical background required; licensure not mandatory
• Proficient in Microsoft Office and able to type at least 35 WPM
• Demonstrates a professional presence both over the phone and in person
• Consistent attendance and reliability are essential
• Ability to manage tasks effectively under pressure
