Assist patients with Chronic Care Management (CCM), Behavioral Health Integration (BHI), and Remote Physiological Monitoring (RPM)
Develop and implement individualized care plans to improve patient outcomes
Required Qualifications
Must reside in the Mountain or Pacific Time Zone
Certification as a Certified Medical Assistant (CMA) or Nationally Certified Pharmacy Technician (CPhT)
2+ years of experience in care coordination or clinical healthcare roles
Experience working with EMR systems
Intermediate to advanced computer skills
