Job Summary

A company is looking for a Care Coordinator I to facilitate client care and support clinicians in a mental health telehealth setting.

Key Responsibilities

• Facilitate the execution of clinician-developed care plans and assist clients with scheduling needs
• Support clinical teams with administrative tasks and ensure effective communication among team members
• Monitor patient progress, ensure follow-up scheduling, and maintain accurate client records

Required Qualifications

• Previous experience in a medical or mental health office, particularly with scheduling and referral coordination
• Working knowledge of Electronic Medical Records Systems
• Experience with Excel, Word, and other computer programs
• Ability to work collaboratively in a team with minimal supervision
• Customer service experience (2 years preferred)
