Job Summary

A company is looking for a Care Coordinator Associate - Authorization Hub.

Key Responsibilities

• Assist in hospital transitions as part of an interdisciplinary care team
• Ensure smooth throughput and care transitions for patients with physical and behavioral healthcare needs
• Utilize multiple data platforms and troubleshoot technology issues as needed

Required Qualifications

• Post-secondary coursework, preferably in a health-related field
• Bachelor's degree highly desirable
• Minimum of 2 years experience in a healthcare setting, preferably as a Health Unit Coordinator or in a clinical role
• Experience in utilization management and community resources desirable
