Supports and communicates with Members to identify needs and provide information on healthcare access and preventative health interventions
Links Members to community resources and assists with scheduling appointments
Maintains accurate documentation of Member interactions in the electronic care management platform
Required Qualifications, Training, and Education
High School Diploma or GED required; Associate's degree preferred
Minimum two (2) years of experience in a healthcare environment required
Minimum one (1) year of telephonic customer service experience required, preferably in healthcare
Medical terminology experience preferred
Training or certification as a medical assistant, home health aide, nursing assistant, or similar healthcare paraprofessional preferred
