Job Summary

A company is looking for a Care Coach I to empower caregivers by providing support and resources for effective caregiving.

Key Responsibilities

• Engage and educate caregivers through phone and digital technology to enhance their wellbeing
• Support various caregiver programs by navigating resources and coordinating care
• Collaborate with the coaching team to address caregiver challenges and document interactions accurately

Required Qualifications

• 2+ years of experience in care/disease management with older adults or individuals with disabilities
• Knowledge of Medicaid, Medicare, and Managed Care systems
• Strong understanding of case management and community service systems
• Fluency in Spanish, both written and spoken
• Ability to multitask and make decisions in a fast-paced environment
