Job Summary

A company is looking for a Clinical Pharmacist Market Support for the Clinical Excellence Team, working remotely.

Key Responsibilities:

• Conduct telephonic patient outreach and provider-facing activities to enhance medication management and adherence
• Develop market business plans to improve STARs measures related to medication adherence and chronic disease management
• Analyze data and provide recommendations to enhance pharmacy-related performance metrics for providers and groups

Required Qualifications:

• Doctor of Pharmacy degree or equivalent experience
• Active, unrestricted pharmacy license in the primary state of residence
• 5+ years of clinical pharmacy practice experience
• 2+ years of experience with Medicare STARs and HEDIS measures
• 2+ years of experience in provider-facing interactions and data analysis
