Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers and ensure optimal healthcare outcomes
• Collaborate with providers to create ongoing service plans and connect members with necessary resources
• Monitor member progress and update care plans as needed, providing education and support to members and their families

Required Qualifications

• Degree from an accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Registered Nurse (RN) state licensure or compact state licensure required
