Job Summary

A company is looking for a Social Worker Care Manager (Hybrid - Philadelphia).

Key Responsibilities

• Support the mental and physical health care of patients on an assigned caseload
• Screen and assess patients for mental health and substance abuse disorders, facilitating engagement and follow-up care
• Document patient progress and treatment recommendations in the EHR and track clinical outcomes

Required Qualifications

• Minimum of MSW
• Preferred Licensure or provisionally licensed
• Behavioral Health experience and knowledge
• Must have a valid driver's license and insured automobile
• Time management skills are essential
