Job Summary

A company is looking for a Clinical Pharmacy Specialist - Ambulatory Care - Oncology Med Management.

Key Responsibilities:

• Provide direct patient care and manage medication for patients with chronic health conditions
• Monitor patient charts to identify and resolve drug-related issues and ensure effective medication management
• Facilitate medication access, assist with prior authorizations, and participate in patient care services and quality improvement initiatives

Required Qualifications:

• Doctorate (PhD or MD) from an accredited pharmacy degree program
• PGY-1 Pharmacy Residency preferred or 3 years of experience as a clinical pharmacist
• Licensure as a Doctor of Pharmacy by the Oklahoma State Board of Pharmacy
• Certificate as a preceptor by the Oklahoma State Board of Pharmacy when eligible
• Board Certified Pharmacotherapy Specialist certification preferred
