NJ Licensed RN Patient Liaison

Job Summary

A company is looking for an RN Liaison.

Key Responsibilities

• Facilitates patient engagement and transitions across the continuum of care with the post-acute interdisciplinary team
• Performs holistic assessments and develops care management plans in collaboration with the care team
• Documents care plans and patient progress accurately in the electronic health record and coordinates community resources

Required Qualifications, Training, and Education

• Bachelor's Degree preferred
• Valid NJ RN License required; NY RN license preferred
• A minimum of 4 years of Care/Case Management experience or comparable clinical experience preferred
• Certified Case Manager (CCM) preferred
• Proficient in Microsoft Office 365 and capable of working remotely
