About the Role:
Impact Life is seeking a compassionate and highly organized Treatment Navigation Coordinator to serve as the first point of contact for individuals seeking behavioral health and recovery support services. The Treatment Navigation Coordinator is responsible for conducting admissions screenings, providing care navigation and telephonic case management, and ensuring that individuals are linked to appropriate internal or external services. This role plays a critical part in client engagement, barrier reduction, and ensuring access to timely support.
Key Responsibilities:
- Conduct initial screenings and assessments to determine eligibility and service needs.
- Maintain ongoing communication with individuals awaiting placement, offering support, guidance, and regular follow-up.
- Provide telephonic care coordination and case management to facilitate access to services and community resources.
- Collaborate with internal teams and external partners to ensure warm handoffs and seamless service transitions.
- Collect, enter, and manage accurate client-level data related to screenings, referrals, linkages, and service outcomes.
- Identify and address barriers to access such as insurance issues, lack of identification, or transportation challenges.
- Maintain detailed follow-up records in case management systems and track engagement efforts.
- Support quality improvement and compliance efforts through timely and accurate documentation and reporting.
- Participate in team meetings, outreach activities, and organizational committees as assigned.
- Comply with all Impact Life policies, procedures, and professional standards.
Qualifications:
- High School Diploma or equivalent required.
- Certified Peer Recovery Specialist (CPRS) or similar credential preferred.
- Bilingual abilities are a plus.
Key Skills:
- Experience in care coordination, admissions/intake, peer support, or community-based services.
- Strong communication skills and comfort providing empathetic telephonic support.
- Knowledge of behavioral health systems, harm reduction principles, and recovery support services.
- High attention to detail with strong skills in data entry, tracking, and case management systems.
