Hawaii Licensed Practical Nurse

Job Summary

A company is looking for a Licensed Practical Nurse (LPN) to support Chronic Care Management and Remote Patient Monitoring programs remotely.

Key Responsibilities:

• Perform comprehensive assessments of patients' medical, psychological, and social needs
• Assist in developing and implementing individualized care plans in collaboration with a multidisciplinary team
• Educate patients on managing chronic diseases and facilitate communication between patients and healthcare team members

Qualifications and Requirements:

• Active Hawaii Licensed Practical Nurse (LPN) license in good standing
• At least 2 years of experience in patient care, care coordination, or chronic disease management
• Proficiency in electronic health records (EHR) systems
• Certification in disease management, case management, or patient education is a plus
• Experience with Chronic Care Management or Remote Patient Monitoring programs is strongly preferred
