Position Overview:
We are seeking a compassionate and skilled Registered Nurse to join a highly rated Assisted Living Facility in Brooklyn, NY. This is a part-time role with a focus on delivering high-quality nursing services in a home care setting.

Qualifications:
- Education: Associate/Bachelor’s Degree in Nursing.
- Licensure: Active New York State Registered Nurse license.
- Experience: Minimum of 6 months clinical experience.
- Technical Skills: Proficiency in home care assessments, care planning, and documentation.
- Soft Skills: Strong communication, empathy, and organizational skills.
Key Responsibilities:
- Perform home care assessments and re-assessments of patients and their environments to determine care needs, utilizing the Oasis assessment tool as required.
- Call the home care coordinator or supervisor with verbal report of initial findings.
- In conjunction with the family and physician and/or authorized practitioner and the facility supervisor, develops and implements the Plan of Care based on the assessment.
- Provide skilled nursing care as outlined in the Plan of Treatment to include the following: nursing services, treatments, and preventive procedures including those requiring substantial specialized skill and ordered by the physician or authorized practitioner.
- Initiate preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety.
- Observe signs and symptoms and changes in the patient's physical or emotional condition and consult with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.
- Report all patient changes and information to the supervisor or home care coordinator on a timely basis.
- Submit a record of visits/hours worked each day.
- Teach, supervise, and counsel the patient and caregivers regarding the nursing care needs and other related problems of the patient at home.
- Document services rendered, changes noted in the patient’s condition, and/or family and home situation, and evaluate patient response to treatment.
- Documentation of all care provided is in accordance with the facility's policies and procedures.
- Participate in Performance Improvement activities.
- Orient and supervise home health aide staff and documents supervision according to the facility's policies and procedures.
- Observe client rights and confidentiality.
- Observe infection control and standard precautions.
- Observe Advance Directives, as indicated.
- If assigned to an adult care facility, complete statistical data.
