Join a care-focused team supporting patients through referral coordination, specialty triage, and access to community care. These are full-time, Monday–Friday, non-bedside RN roles — ideal for experienced nurses ready to transition into case management and care coordination.
We are hiring multiple Registered Nurses (RNs) to support two mission-critical teams within the Health Facility:
- Coordinate and clinically review referrals for external (community) care
- Ensure referral completeness and determine medical necessity
- Review incoming medical records and communicate with providers
- Facilitate authorizations and track community provider care status
- Triage specialty consults and ensure clinical appropriateness
- Call patients to explain specialty care options and help them choose care settings (in-facility vs. community)
- Serve as clinical liaisons to internal/external providers
- Collaborate with providers and support teams to streamline consults
- Active, unrestricted RN license in at least one U.S. state, territory, or the District of Columbia
- Graduation from an accredited nursing program approved by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
A bachelor’s degree in nursing (BSN) is not required, but preferred.
- Minimum of 2 years of direct patient care experience within the last 3 years
- Current Basic Life Support (BLS) certification
- U.S. Citizenship
- Proficiency in English (spoken and written)
- Must meet credentialing and privileging requirements before start
- BSN degree
- Prior experience in case management, care coordination, utilization review, or community health
- Familiarity with referral workflows and specialty triage
- Monday–Friday schedule (no weekends or holidays)

- Onboarding includes credentialing, physical exam, immunizations, and CME tracking
- Active, unrestricted RN license in at least one U.S. state, territory, or the District of Columbia
- Graduation from an accredited nursing program approved by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
