Nurse Practitioner/Physician Assistant - SSRCC - 10462

The Nurse Practitioner (NP)/Physician Assistant (PA), is responsible for providing primary health care to individuals and families who will be accessing care at Stout Street Health clinic, Stout Street  Recuperative Care Center (SSRCC), and our other clinical sites  This position is for a  NP/PA for 40 hours per week, that will include 1 weekend day as part of their work week. The NP/PA would provide high-quality acute, chronic, and preventive care across the full age span, and works collaboratively with the SSHC multidisciplinary integrated health team.  

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date.  The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance.  Please note that a start date will be scheduled once credentialing and privileging is complete.

• Collaborates and consults effectively and bi-directionally with all CCH staff and identifies and addresses the complex issues which may adversely impact the health of patients.  
• Provides clinical precepting opportunities for providers-in-training as scheduling allows.  
• Maintains Continuing Medical Education, license (s), and CCH Credentialing and Privileging requirements always.     
• Delivers care across CCH sites which may include outpatient clinic sites, the mobile clinic, Stout Street Recuperative Care, street outreach or home visits.  
• Weekend /evening/Holiday coverage is required. 
• Performs other duties as assigned.  

• 2 or more years of experience required for this job position. 

• A Master of Science or Doctorate degree in Nursing from a program accredited by the American Nurse Association or the American Association of Nurse Practitioners.  
• Current registration (or ability to obtain) in the State of Colorado as an Advanced Practice Nurse.  
• Certification as a Nurse Practitioner from the American Nurse's Credentialing Center.

• Graduation from a program accredited by the American Academy of Physician Assistants.   
• Current licensure (or ability to obtain) with the State of Colorado to practice as a Physician Assistant.   
• Current certification with the National Commission of Certification of Physician Assistants.
