Job Summary

A company is looking for a Clinical Pharmacist to perform professional duties related to medication management.

Key Responsibilities:

• Assess medication regimens for indication, effectiveness, safety, and compliance
• Provide patient-centered care to improve clinical outcomes and communicate care plans effectively
• Engage with patients and collaborate with healthcare providers to optimize therapy

Required Qualifications:

• Bachelor's Degree in Pharmacy or PharmD
• Current unrestricted active pharmacist license in the US
• Multi-state pharmacist licensure is preferred
• Residency and/or Board Certification is preferred
