Job Summary

Key Responsibilities

• Provide indirect oversight of Collaborative Care Clinicians' performance and support administrative workflows
• Manage a caseload of up to 45 patients, customizing treatment plans and coordinating care with primary care providers
• Conduct patient assessments, monitor clinical symptoms, and document progress in electronic health records

Required Qualifications

• Master's degree in social work, counseling, or a related field
• Independently licensed in CA or eligible for independent licensure within 6 months
• Minimum of 2+ years of clinical experience
• Proficiency in evidence-based treatment approaches and behavioral health tools
• Technical proficiency in EHRs and ability to learn new technology quickly
