Physical Therapist - Full-time

Responsibilities

  • Conduct assessments and develop individualized treatment plans for patients based on their physical needs and abilities.
  • Provide in-home physical therapy services to patients in accordance with their treatment plans.
  • Continuously assess and adjust treatment plans as needed to ensure progress towards patient goals.
  • Communicate regularly with patients, family members, and other healthcare professionals to coordinate care and ensure effective treatment.
  • Document all patient interactions, assessments, and treatment plans according to AdvisaCare standards.
  • Adhere to all state and federal laws and regulations related to physical therapy practice.
  • Maintain accurate records and reports of patient progress and treatment outcomes.
  • Current licensure as a Physical Therapist in the state of Michigan.
  • A minimum of one year of PT experience, ideally in a home health care setting, is preferred.
  • Strong understanding of physical therapy best practices and treatment methodologies.
  • Excellent communication and interpersonal skills, with the ability to connect with patients and their families.
  • Ability to work independently and manage a caseload of patients effectively.
  • Must have a valid driver's license and reliable transportation.
